Qualifying Abuse Checklist

Use this checklist to determine the types of supporting evidence you can file with your petition to prove this criteria. You do not need to file all of these items with your petition to prove this criteria; one of these should be sufficient, but the more evidence you can file, the stronger your case will be. 

  • Your affidavit discussing your abuse; 
  • Reports from police, judges and other court officials;
  • Reports and affidavits from medical personnel, school officials, clergy, social workers, and other social service agencies; 
  • Orders of protection or other court findings of abuse against abusive spouse or relation; 
  • Documentation that you sought assistance for abuse, such as a record of residence in domestic violence shelter, or participation in a domestic violence support group; 
  • Affidavits from family, friends and neighbors with knowledge of abuse; or 
  • Photos showing impact of abuse.
